401(k) Salary Reduction Agreement & Beneficiary Designation/Change Form
Employee 401(k) Enrollment/Change Form
This form should be completed for the following reasons: to change % or amount contributing, change beneficiary(ies) or to change investments (if applicable).

Please select one of the following forms to complete.  You may need to contact your HR department if you are unsure which form you need.

Termination/Retirement Withdrawal Form
Employee Termination/Retirement Form
This form should be completed by employees who have retired, quit or had their employment terminated. Print and complete this form, then return to your HR Department.


In-Service Withdrawal Form
Employee In-Service Withdrawal Form
In most cases this form should only be completed by employees who have attained age 59 ½ and are 100% vested for all sources in their account.  You will need to check with your HR department before completing this form to see if you are eligible for an in-service withdrawal.  If you are eligible, print and complete this form, then return to your HR Department.

Hardship Withdrawal Application
Employee Hardship Withdrawal Application
In order to qualify for a hardship withdrawal you must meet one of the criteria listed on the withdrawal form and provide proof of your financial hardship to your employer.  Not all Retirement Plans allow for hardship withdrawals.  Please check with your HR Department before completing this form to see if you qualify and if your Employer’s Plan allows for hardship withdrawals.  If so, print and complete this form, then return to your HR Department.

Loan Request
Employee Loan Request Form
Check with your HR Department to see if your Employer’s Plan allows for loans before completing a Loan Request. If so, print and complete this form, then return to your HR Department.


Death Benefit Withdrawal
Beneficiary Death Benefit Withdrawal Form
This form should be completed by Beneficiary(ies) of a participant who has passed away.  In some cases it may be necessary to include a copy of the Death Certificate when returning this completed form to the HR Department of the deceased’s Employer.
