| Becoming an Approved Program Provider If you are a TSA vendor and are interested in providing services to Hawaii DOE employees, please contact the Program's third-party administrator, National Benefit Services, LLC (NBS) at HawaiiDOE403b@nbsbenefits.com or 800.274.0503 ext. 240. Vendors must enter into the NBS 403(b) Provider/Information Sharing Agreement and complete the "Essential Information Relating to Facilities - Form 500a" to request approval from the State of Hawaii Department of Education. |

Offering Mutual Funds to Program Participants If you are an existing Program vendor and wish to offer mutual fund custodial accounts to participants in addition to annuity products, a 403(b) Provider/Information Sharing Agreement (Custodian) must be submitted to NBS. If you are not an existing Program vendor you must also submit the Essential Information Relating to Facilities — Form 500a and obtain approval from the State of Hawaii Department of Education. |
